AKC Title Application Form

AKC will recognize NADD Dock Diving Titles through the AKC Title Recognition Program. You must complete the form  (download link below), include a copy of your NADD title and submit with $30 to the AKC. AKC will put the title on your pedigree and issue you an AKC Title Certificate.
